The work
Transforming Systems
When systems of care are built to meet the needs of the most marginalized people in our communities then we move closer to meeting the needs of all people in our communities. Small, yet achievable adjustments at every level of a system will yield significant change. A person centered system will improve health outcomes and build trust with the people you serve. This alignment also improves retention and job satisfaction among staff.
Liberating Resources
When our systems meet people where they’re at and we screen people in, then the resources that are available can flow more freely and can be put to work improving health outcomes. Currently, many systems screen people out and when this happens, resources are withheld and do not reach those who need them the most.
Meaningful Change
With aligned systems and resources, meaningful change can begin to take place. This is the beginning, not the end of the process. Meaningful change requires a long term commitment to collaborative care, autonomy, and harm reduction. Expected outcomes include trust building, greater engagement, and positive relationships that yield critical insight to the way you deliver services. This is the work.
